Steps for CPAs Who Lost Their Licensure Credentials in the WTC Tragedy The State Education Department's Office of the Professions has adopted the following procedure for those licensees who lost their licensure credentials in the World Trade Center tragedy. I wanted to inform you so that you could disseminate this information to your membership. Replacement of Lost Documents: Due to the disaster in New York City, the Office of the Professions in the State Education Department has set up a special procedure to handle Licensee's calls requesting replacement(s) for documents (Registration Certificate and/or a License Parchment) destroyed during the terrorist attack in New York on September 11, 2001. All replacement requests due to this disaster should be forwarded to the Office of the Professions' Records and Archives unit at: 518-474-3817 ext. 380. They will handle replacing both documents. The Records and Archives Unit will produce an individualized form for the licensee to complete that will take care of requests for both registration documents and license parchments. This form is specifically to be used for current registrants who have lost their licensure documents through the destruction or closing down of property in the terrorist attacks on the World Trade Center. The Records and Archives unit will fax the form to the licensee/registrant and have the person fill it out and fax it back (E-mail requests are not sufficient because there would be no signature). There will be no fee to the licensee/registrant for the replacement parchment or registration certificate. Replacement parchment and registration documents will be mailed to licensees within three business days from the time of their faxing the request form to the Records and Archives unit.
